Community Arts Awards Nomination Form

All Community Arts Awards nominations must be received by the Arts Council of New Orleans no later than Monday, NOVEMBER 30, 2009. Nominations received after this date cannot be considered. Current members of the Arts Council’s Board of Directors are ineligible for the award.

Type or print all information. Do not include attachments of additional materials such as samples of work or biographies.

There is no limit to the number of nominations that may be submitted by an individual or organization; however, a separate email or letter must be used for each nomination. The number of nominations submitted does not increase the nominee’s chance of receiving an award.
